Most drugs are pills you pick up at a pharmacy — those run through Part D. But drugs a clinician has to inject or infuse — chemo, biologics, the new cell and gene therapies — are billed under Part B. For those, Medicare pays a set rate: the drug's Average Sales Price plus 6%, published every quarter. Here it is, sorted by the most expensive per unit.

What Medicare pays, per billing unit
The rate is per billing unit — often a milligram, but for a cell or gene therapy the unit is a single patient dose, which is why they sit at the top.

How to read it: “Medicare pays” is the Part B payment limit (ASP + 6%) for one billing unit. What a patient with Medicare owes is usually 20% of that, unless a supplement or other coverage picks it up. This is the allowed rate, not what any single hospital paid.
